That's according to a HIQA report into conditions at the centre following two unannounced visits earlier this year.
The report also found there was “ineffective leadership, governance and management arrangements” at the centre, which led to continual failings which put residence at an unacceptable risk.

It also finds that it was difficult to tell who was responsible for managing the campus and that there were no efficient procedures to take on extra staff whenever shortages occurred.
