Written warnings were issued to some of the 30 staff involved in her care, before she died of an infection at University Hospital Galway in October 2012 following a miscarriage.

Other workers were given 'pre-procedural' informal counselling around improving their work, and 21 of her care staff were found to have 'no case to answer'.

The HSE has not identified which staff members were disciplined for privacy reasons.
