HIQA report due on Portlaoise Hospital.

It follows the deaths of five babies in similar circumstances at the hospital including that of baby Mary Kate Kelly from Borrisoleigh.

This report from the Health Information and Quality Authority is expected to heavily criticise the HSE.

The Irish Times reports it will find that all levels of HSE and hospital management were aware of known patient safety risks at the hospital, but failed to act.

It's understood criticisms contained in a previously leaked draft report remain, despite a threat by Director General of the HSE Tony O'Brien to injunct publication if changes were not made.

The 200 page document – which will be launched later this morning – has already been approved by the board of HIQA and has been sent to the Health Minister.
