A Tipperary business was among ten food outlets shut down by the FSAI last month.
Other closure orders were served in Dublin, Kerry, Roscommon, Carlow and Limerick.
An FSAI inspector visited the Shake Dog restaurant at the Showgrounds in Clonmel on May 14th.
Concerns were raised about the failure to implement Hazard Analysis and Critical Control Point based procedures.
Examples were given of cooked bacon, raw chicken, garlic mayonnaise and raw sausage all being held at temperatures above the critical limit of 5 degrees.
Equipment was also found to have evidence of food residue while a wash hand basin behind the counter in the milkshake – drinks area was covered with plastic film while stagnant water was pooling in the basin.
The closure order was lifted on May 19th.
